INSTRUCTIONS

• Age: For admission to class-I, 5 years plus as on 31st May, 2018, should not complete 6 years (i.e., born between 01-06-2012 and 31-05-2013).
• At the time of registration, date of birth certificate in ORIGINAL should be produced for verification and a Xerox copy should be submitted for record. Registration will not be done without the date of birth certificate from the Municipal Authorities or the Mandal Revenue Officer with his official stamp. Certificate/s from Doctor/s and Maternity House, non-availability certificates from Municipal Authorites or Certificates from other school/s will not be accepted.
• The parents are also requested to produce attested Xerox copies of the residence proof/s.
• Mere registration of the application does not guarantee a seat.
• Un-registered application/s will not be considered for admission under any circumstances.
• Canvassing of any sort will be considered as a disqualification.
• Non-compliance with the requirement and submission of factually incorrect information will result in refusal of admission.

If the parent is a Member of Jubilee Hills Co-operative House Building Society Ltd., attested Xerox copies of the Membership proof should be enclosed.

ADMISSION PROGRAMME FOR CLASS-I FOR THE YEAR 2018-2019

• The prescribed application form may be downloaded from the website of the school from 19th February, 2018 (9.00 a.m.) to 21st February, 2018 (4.00 p.m.)
• The application form, duly filled in and supported with copies of prescribed documents, may be registered at the school Office counter on payment of Registration fee of Rs.500/-(cash) from 20th February, 2018 to 22nd February, 2018 (from 9.30 a.m. to 12.30 p.m. on all working days). NO EXTENSION WILL BE ALLOWED either for downloading application form or processing of application.
• Enclosures to be attached with the application:
  1. Copy of Date of Birth Certificate
  2. Copy of residence proof
  3. Copy of Membership proof, in case the parent is a Member of Jubilee Hills Co-Operative House Building Society Ltd.,
  4. Copy of relevant document (study/Transfer Certificate) regarding their studies in the school in respect of parents who are ex-students.
  5. Copy of Bonafide Certificate, in case of sibling students.
  6. Copy of Student’s Aadhar card.
  7. Copy of valid VISA for the period of their study in the school, in case of a child having foreign citizenship.
  8. Authorisation letter from the parent with the photograph of guardian duly pasted on it should be enclosed.

Other important instructions to parents:

• Application should be signed by both the parents.
• Latest Passport size photographs of the child and parents should be submitted (not more than a month old photos).
• If the child is staying with the guardian, specific authorization letter from both the parents mentioning the name, contact number and complete address should be submitted.
• To consider admission for a few seats, draw of lots will be conducted on 04-03-2018 (Sunday) at 9.30 a.m. in the school premises.
